Immunization Policy

Purpose
This policy establishes guidelines for immunizations required for all Nucleus University School of
Dentistry (NUSOD) students. It ensures that the students provide evidence of appropriate
vaccination, which assists in safeguarding students' health and reducing the incidence of vaccine-
preventable diseases in the community.

Scope
All university students enrolled in a Dentistry program of study leading to a Doctor of Dental
Surgery (DDS) degree, as well as faculty and staff of NUSOD.

Policy and Procedures
Health and Immunization Requirements
- All incoming Doctor of Dental Surgery students must complete a Student Health Clearance
Certificate form from a healthcare provider before matriculation and clinical clerkship. Ensure
that the healthcare provider signs the form and then return the form to the Office of
Admissions before matriculation. All documents are to be emailed to info@nusom.org.
- To ensure on-time registration, students must complete all necessary vaccination testing and
submit their vaccination history and documentation before arriving at campus.
- Students can only register or participate in clinical activities once the Health Clearance
Certificate form is completed and on file. Students re-enrolling after a lapse in attendance of
one year or more or who have been readmitted into the program must submit an updated
form.

Health History and Immunization Form for Incoming Students
All students must submit the Health Clearance Certificate form as part of their application
process to the program. The Admissions Committee reviews this information, and the
Office of the Registrar maintains all records. All medical information is strictly
confidential and cannot be disclosed without the student’s authorization except in medical
emergencies.

Required Immunization
Before completing registration, all incoming students must provide documentation of the
required immunizations and tests.
- Mumps, Measles and Rubella (MMR):
- All students born after January 1st, 1957, must provide evidence of immunization
against Mumps, Measles (Rubeola), and Rubella (German Measles). Such evidence
can be provided through vaccination against all three diseases with a combined
vaccine (MMR) or individual vaccinations against each disease. Alternatively,
students can show laboratory evidence of immunity to all three diseases
- A record of 2 MMR vaccines
- Dose one (1) – at twelve (12) months of age or older
- Dose two (2) – at least one month after dose one (1)
- Positive Antibody titer for immunity to Mumps, Measles and Rubella
- The presence of a medical history of disease alone is not acceptable
- A copy of the lab report must be attached to this form.
- Hepatitis B (HepB):
- All students must provide evidence of immunization against Hepatitis B. Such
evidence can be provided through vaccination against Hepatitis B with a combined
vaccine Hepatitis A and Hepatitis B vaccine (HepA-HepB) or a Single Antigen
Hepatitis B vaccine
- A record of Single Antigen Hepatitis B vaccine series three (3) doses in total
- Dose one (1)
- Dose two (2) - at least (one) 1 month after dose one (1)
- Dose three (3) - Six (6) months after dose one (1)
- If the vaccination series is complete, a Hepatitis B Surface Antibody (HBsAb) titer
must be performed at least four (4) weeks after the final dose
- If the HepBsAb titer is (<10 IU/ML) negative, please obtain a booster dose
- If the HepBsAb titer is low or not strongly positive and HepBcAb and HepBsAg are
both negative, then (re)vaccination against hepatitis B is suggested.
- If the HepBsAb, HepBcAb, and HepBsAg are all negative, then (re)vaccination
against hepatitis B is mandatory
- A copy of the lab report must be attached to this form.
- Poliomyelitis (IPV):
- Two (2) doses of IPV or
- One (1) dose of adult IPV booster if received oral polio vaccine as a child.
- Tetanus/Diphtheria/Acellular Pertussis (Tdap or Td):
- One (1) dose of adult Tdap (Tetanus, diphtheria and acellular pertussis (whooping
cough) within ten (10) years
- If the last Tdap was more than ten (10) years ago, a Tetanus-Diphtheria (Td)
booster dose is required.
- Tuberculosis (TB):
- Test for Tuberculosis is required before enrollment
- Documentation of having received the Bacille Calmette-Guérin (BCG) vaccine does
not preclude the need to test for Tuberculosis (TB)
- A 2-StepTuberculin Skin Test (TST) or MANTOUX skin test before the beginning
of classes
- Positive PPD is required to have a negative chest X-ray
- The health provider’s report stating that the student does not have a communicable
disease if the student has a positive test result.
- The health provider’s report must accompany the TB test report and be attached to
this form.
- Varicella (VAR):
- Two (2) doses of varicella (Chickenpox) vaccine, at least one (1) month apart or
- A positive serological test for immunity to Varicella (chickenpox)
- The presence of a medical history of disease alone is not acceptable.
- A copy of the lab report must be attached to this form.
- Novel coronavirus disease 2019 (COVID-19)
- Documentation of having received COVID-19 vaccination
- Two (2) doses given at least three (3) weeks apart of Pfizer-BioNTech/
Oxford-AstraZeneca/Moderna, Sinopharm-BIBB/ CoronaVac or
- One (1) dose of Johnson & Johnson
- A copy of your COVID-19 vaccination record must be attached to this form.
- Influenza:
- One (1) dose of Annual influenza vaccination
- Hepatitis C (HepC):
- Test for Hepatitis C Antibody is required
- A copy of the lab report must be attached to this form.

Clinical Students
Students must ensure they have up-to-date immunizations before the commencement of the
clinical clerkship. Immunization requirements may differ depending on the hospital and location
of the clerkship. Therefore, the students must ensure they adhere to the immunization
requirements of the hospitals where they are scheduled to undertake their clerkship rotations.

Vaccination Waiver
The health provider can issue a vaccination waiver if the vaccine is not recommended for medical
reasons. Should a medical contraindication to the immunization requirements exist, a letter with
a medical report from the health provider must be submitted. The student and the healthcare
provider must sign the letter. The letter is to be submitted to the Dean of Student affairs for review
and approval.
