At Nucleus University School of Dentistry (NUSOD), we uphold the rights of students to access their educational records while maintaining confidentiality and privacy. The purpose of this policy is to outline the procedures for students to inspect, request corrections to, and limit disclosures of their educational records in accordance with the Family Educational Rights and Privacy Act (FERPA).
NUSOD adheres to the regulations stipulated by FERPA, ensuring that students can exercise their rights concerning their educational records:
Students may request to inspect their educational records by submitting a written request to the appropriate Dean.
Upon receiving the request, the Dean will facilitate the inspection within 45 days and notify the student of the time and location for review.
If students identify inaccuracies in their educational records, they have the right to request corrections by submitting a written request to the Appropriate Dean, specifying the details of the correction needed.
If the requested amendment is denied, the student will be informed of their right to a hearing, where additional information will be provided.
Students have the right to consent to the disclosure of personally identifiable information contained in their educational records, with certain exceptions as outlined in FERPA.
Disclosure may occur to school officials with legitimate educational interest or as authorized by FERPA regulations.
NUSOD may disclose directory information, including name, address, and dates of attendance, without prior consent from the student.
Students who wish to restrict the release of directory information must submit a written request to the Registrar’s Office.
This policy applies to all members of the NUSOD community, including students, Deans, Registrar, System chairs, and Faculty.
NUSOD provides official transcripts upon formal request, containing comprehensive information about a student’s academic performance and achievements. The following procedures govern transcript requests:
Students must complete a Transcript Request Form available through the Learning Management System.
Official transcripts will be issued within two weeks of receipt of a completed request form and payment of applicable fees.
Transcripts include essential details such as student name, date of birth, courses taken, grades achieved, and degree awarded, among others.
Students may request official transcripts to be sent to other educational institutions or organizations as needed.
A fee of $100 USD is applicable for each official transcript request.
NUSOD maintains strict confidentiality regarding student transcripts and ensures compliance with regulations.
Any misrepresentation or falsification of documents will be subject to disciplinary action.
